The Transition Age Youth Services Teams works to support foster youth between the ages of 16 and 25 who are struggling with mental and emotional health issues. Through outreach efforts and a variety specialty services, the team's goal is to help these youths learn to self-manage their symptoms, so they may successfully transition into independence.
Specialty Services
The program's services are designed specifically to meet the needs and goals of each youth. Working with a team of clinicians and case managers, an individual plan is built to help each youth focus on their own emotional and physical needs.
- Counseling/therapy
- Drug and alcohol addiction treatment
- Help securing financial and health benefits
- Assistance with accessing medical, dental, and vision services
- Assistance with searching for safe, affordable housing options
- Help finding transportation options
- Help with job searching or enrolling in vocational training or education classes
- Food and clothing resource services.
Eligibility
A person must:
- Be between the ages of 16 and 25
- Have a current mental health or emotional health disability
- Be a current or emancipated foster youth or a current or form juvenile justice client
